This Notice describes how medical information about you may be used and disclosed and how you can get access to this information under the Health Insurance Portability and Accountability Act (HIPAA). Please review it carefully.
Uses and disclosures for treatment, payment, and healthcare operations
We use your protected health information (PHI) to deliver clinical care, coordinate with pharmacy partners, bill for services, and operate the practice (e.g., quality improvement). We will obtain your written authorization before using or disclosing PHI for any other purpose.
Your rights
You have the right to: (1) inspect and copy your PHI, (2) request amendments, (3) request an accounting of disclosures, (4) request restrictions on certain uses, (5) request confidential communication, and (6) file a complaint without fear of retaliation.
Our responsibilities
We are required by law to maintain the privacy and security of your PHI, provide this Notice, and follow the terms of the Notice currently in effect. We will notify you promptly if a breach occurs that may have compromised your information.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing.
