Telehealth Informed Consent

Last updated: April 2026 · Draft pre-launch

By using Wholelift, you consent to receive healthcare services via telehealth. This document describes what telehealth care looks like at Wholelift and what to expect.

What telehealth means here

Our services are asynchronous — you complete an intake, your provider reviews it, and care is delivered through secure messaging. In some cases, your provider may request a synchronous video visit, additional information, or in-person evaluation before prescribing.

Benefits

Convenient access to licensed clinicians, no travel required, reduced wait times, and discreet delivery of medications. Many patients find telehealth particularly valuable for chronic conditions and medication management.

Risks and limitations

Telehealth has limitations. Without in-person examination, some conditions cannot be fully evaluated. Technical issues may delay care. Your provider may determine that your condition is not appropriate for telehealth and refer you elsewhere.

Your rights

You may withdraw consent for telehealth at any time without affecting your right to future care. You will always have the option to seek in-person care instead. All communication is protected under HIPAA.

Emergencies

Wholelift is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
